I. Basic Documents Required
Nominees must submit the following on or before the deadline set by the FSTPU for the submission of nomination and the required documents:
A. Nomination letter signed by the Department Secretary or head of the agency / university or entity or the duly authorized official, addressed to:
PASTOR Z. GUIAO
Director General
Technical Education and Skills Development Authority
East Service Road, South Superhighway,
Taguig City, Philippines
Attention: FOREIGN SCHOLARSHIP and TRAINING PROGRAM UNIT
The nomination letter should state why the candidate is being nominated and that the nominee, when accepted, will continue to be paid his/ her salary and other entitlements due him/her while in the course of training.
B. Updated Personal Data Sheet including list of training and seminars attended
C. Statement of Present Actual Duties and Responsibilities RELEVANT to the course nominated in signed by the Immediate Supervisor
D. Certification from the Head of the Human Resources Department or Personnel Manager on the following:
1.That the nominee has no pending administrative and /or criminal case
2.That the nominee has no pending scholarship nomination and has already rendered the required service obligation for a scholarship previously enjoyed if applicable.
3.That the nominee has obtained at least a Very Satisfactory performance rating for two consecutive periods preceding the nomination.
E. Certificate of non-withdrawal signed by the nominee and attested by the HRD Head
F. Certification from a medical doctor that the nominee is fit to travel and undergo foreign trainingwith the Laboratory Examination result attach.
